Chronic Disease Education and Management

 

The Coumadin Clinic is for patients who take anticoagulation medication. Our clinic is staffed by nurses who have been trained in anticoagulation care. These nurses work as a team with primary care providers to oversee each patient’s treatment.

The Coumadin Clinic’s hours of operation are:

• Monday 8:00 a.m. to 1:00 p.m. and 2:00 p.m. to 4:00 p.m.
• Tuesday 8:00 a.m. to 1:00 p.m. and 2:00 p.m. to 4:00 p.m.
• Wednesday 8:00 a.m. to 1:00 p.m. and 5:00 p.m. to 9:00 p.m.
• Thursday 8:00 a.m. to 1:00 p.m.
• Friday 8:00 a.m. to 1:00 p.m
