Eligibility and Insurance

The Elder Service Plan is available to individuals 55 years of age or older who live in East Boston, Winthrop, Everett, Chelsea, or Revere, qualify for nursing facility level of care as determined by the Commonwealth of Massachusetts, and have the ability to live safely in the community with services provided by the Elder Service Plan.

As an insurer, the Elder Service Plan pays for all medically necessary services covered under Medicare and Medicaid regardless of your original insurance, when approved by your health care team.

Once enrolled in the Elder Service Plan, all care and services, with the exception of emergency care must be approved by the PACE health care team, and provided through the Elder Service Plan. These provisions are known as “lock-in,” remain in effect while enrolled. Participants may disenroll from the Elder Service Plan at any time.

The Medical Secretary here is the best thing in my life. She coordinates my transportation and medical appointments, and gives me my paperwork a day in advance. If I get a medical bill, I bring it to her. Every month they give me a box with all my medications, and have another ready the next month. I don't have to worry about anything.”
