Each PACE Center provides integrated medical, rehabilitation, geriatric case management, and adult day health services all in one location.

At a PACE Day Health Center, participants can spend the day in a warm, friendly atmosphere filled with friends and activities that will help keep them agile and alert. PACE Centers offer onsite:

• Medical care and rehabilitation services
• Productive activities and exercises
• Nurses to monitor health and medication
• Occupational, physical, and speech therapy
• Hot meals and snacks
• Assistance with personal care and activities of daily living
• Medication administration
