Our Program

 

Elder Service Plan participants receive personalized care based on a program that is as individual as they are.

Our interdisciplinary team of physicians and specialists, nurses, social workers, therapists, and others work together to provide comprehensive medical services and support. Personalized care plans are developed in close collaboration with each Elder Service Plan participant and their families.

By taking care of all health care needs under the Elder Service Plan/PACE umbrella, participants no longer have to worry about unnecessary repeat testing or transferring records. All non-emergency services are provided by the Elder Service Plan and its network of providers. A partial list of PACE services include the following.*

Families and caregivers receive:

• Assurance that their loved one is in a safe and supervised environment engaged in stimulating activities when at the PACE Center
• Caregiver support

Participants receive:

• Adult Day Health services for individuals with and without cognitive impairment
• Dental and vision care, including eyeglasses
• In-home assistance with personal care needs such as bathing, dressing, and meal preparation as needed
• Inpatient hospitalization care
• 24-hour access to PACE medical staff
• Medical supplies and equipment
• Medically necessary over-the-counter health care items
• Medically necessary prescription drug coverage
• Medication management
• Nursing care
• Nutrition evaluation and guidance
• Other services as prescribed by the interdisciplinary team
• Physical, occupational, and speech therapy
• Primary and specialty medical services
• Recreational therapy and activities suited to individual interests
• Medically necessary transportation

*In some cases, there may be a monthly premium required based on your income or assets. All services must be provided by or authorized by the PACE interdisciplinary team (except emergency services). PACE participants my be held liable for costs for unauthorized or out of PACE program agreement services.
