Elder Service Plan

Participants in our Elder Service Plan love their independence.

The Elder Service Plan helps seniors stay in our community and live in their own homes, for as long as possible. We provide the care and support that each participant needs. Our integrated plan is the ideal solution for seniors and their families who seek high-quality care that values their personal wishes and way of life. As a result of our program, our elders are living among us, with dignity and respect in the community that they call home.

We believe that well-being is best achieved through a customized blend of support, social services, and outstanding medical care. Our team of experienced professionals works together to provide seamless attention and treatment, in consultation with each senior and their family. This interdisciplinary team is the heart of our program.

Whether by providing transportation to a doctor’s appointment, dental check-up, or a visit to our Adult Day Health Centers for medical oversight and socialization, we serve a wide range of needs. If at-home care or in-patient treatment are appropriate, we provide those services too—with the same commitment to high-quality, customized care.

The Elder Service Plan is available to individuals 55 years of age or older who live in East Boston, Winthrop, Chelsea, or Revere, qualify for nursing-facility level of care as determined by the Commonwealth of Massachusetts, and have the ability to live safely in the community. Medicare, Medicaid, and private payments are accepted. For members of the Elder Service Plan, prescription drugs are covered in full. There are no additional costs to receive the medications that you need to stay healthy.

Established in 1990, this plan is supported through a PACE program agreement with the Centers for Medicare & Medicaid Services, MassHealth, and the East Boston Neighborhood Health Center.
