The Elder Service Plan helps seniors stay in our community and live in their own homes, for as long as possible. As part of the national Program of All-Inclusive Care for the Elderly (PACE) network, we provide the care and support that each participant needs.
At the Elder Service Plan we believe that caring for an individual is a partnership among the senior, his or her family, and the PACE health care team. This partnership is at the heart of our program. The East Boston Elder Service Plan is the ideal solution for seniors and families who want an alternative to nursing home care, but who need a skilled and experienced care partner to identify and arrange for the right combination of medical, social, recreation, rehabilitation, home care, and other services to keep a loved one at home. We even provide medications without co-pay and transportation to the PACE Day Health Center and medical appointments. The Elder Service Plan provides services seamlessly because we are both the health insurer and a medical provider. By giving the Elder Service Plan’s health care team control of their health care dollars, we can partner with our participants and their families to ensure that our participants receive the right care in the right place at the right time. CMS Approval H2223-2009-003; 08/20/2009. The East Boston Elder Service Plan is comprehensive. Participants of the Elder Service Plan must receive all necessary health care, including primary care and specialist physician services (other than emergency services), from the East Boston Elder Service Plan or from an entity authorized by the East Boston Elder Service Plan. Participants may be fully and personally liable for the costs of unauthorized or out-of-program services. You may request disenrollment from the East Boston Elder Service Plan at any time. Your effective date of disenrollment will be the first day of the month following receipt of your request. Please be aware that you cannot disenroll from the Elder Service Plan at a Social Security office. |
