And so does your personal medical information.The East Boston Neighborhood Health Center maintains medical records for all patients who receive treatment or services at our facilities. Most current medical record information is maintained electronically on our computer system. In 1998, we implemented EpicCare, a tier-one electronic medical records system that integrates patient care, registration, scheduling, and billing. Our years of experience with this powerful platform mean that its capabilities are now fully integrated and continually updated. Its benefits are clear throughout the health center. Most importantly, our electronic medical record system improves the care we provide to you, our patient, since your health information—including test results—is readily available to your provider so that he or she can make timely decisions about your treatment. We are located on the first floor of 59 Meridian Street and are open from 8:30 a.m. to 4:30 p.m., Monday through Friday. You can reach us in person or by calling 617-568-4539. Frequently asked questions about the release of medical information: How do I request a copy of my medical record? Requests for a copy of your medical record should be made by contacting the Medical Records department release of information staff at 617-568-4539. Since under federal law there are specific requirements regarding the information that must be contained in an authorization for release of medical information, you will be asked to complete and sign a form before we process your request. You will also need to provide a photo ID so that we can verify your identity. How long will it take to receive a copy of my medical record? Usually it takes no more than 10 business days to obtain a complete copy of a medical record. If you need a copy sooner, let the release of information staff know so that every effort can be made to meet your needs. When I request my medical record, do I receive the original record or a copy? When you request your medical record, the health center makes you a copy. We are required by Commonwealth of Massachusetts law to keep your original medical record on file for 20 years after the last time you were treated here. Can I send someone else to pick up a copy of my medical record? Without your specific written authorization, we are not allowed to give your record to a family member, friend, or anyone other than you. If you have already completed an authorization for release of information form and want someone else to pick up your record, you will need to provide written authorization for us to release your record to that person. If my lawyer needs a copy of my medical record, should I pick up the copy myself? Your lawyer should request the copy of your medical record, since he or she may want the record to be certified as accurate and complete. We do not certify medical records that are picked up by the patient or other persons. In order for us to send a copy of your medical record to your lawyer, we need a written authorization from you or your legally recognized representative. We can provide this authorization form if needed. What is the process for a doctor to fill out and sign forms for me (such as those needed for the MBTA, Family Medical Leave Act requests, and other purposes)? If you need your provider to complete a form, please make your request at the Medical Records department. The form is then sent to your provider, and sent back to us when complete. This process usually takes one to two weeks. When the form is ready for pick up, the Medical Record department staff will telephone you to let you know. |
