YOUR MEDICAL INFORMATION RIGHTS

The medical record is the property of the health center, but you or your legally recognized representative has the right to:

• Obtain a paper copy of this notice upon request
• Request a restriction on some uses and disclosures of the information contained in your medical record
• Obtain a copy of your medical record
• Request to make an amendment to your medical record
• Obtain a list of people/facilities other than the health center or its business associates who have seen or received a paper copy of your medical record
• Request communication of your medical information (such as test results or appointments) by alternative means than those the health center uses
• Revoke your authorization to use or disclose medical information except in cases where information has already been used or disclosed upon your previous authorization
