Living with diabetes is not always easy. The good news is that you can improve your well-being by taking an active role in managing your health. Learning about diabetes and finding new ways to take care of yourself will help you avoid the complications of this disease, and ensure that your future is as healthy as possible.

CDEM offers a Bilingual Diabetes Education and Management Program that emphasizes education and self-management, in Spanish and English. We help patients manage their disease through good nutrition and exercise, and the use of glucometers and medications. Teaching is provided by our diabetes nurse care manager and our nutritionist. Our nurse practitioner and physician assistant work with patients and their doctors to find the medications that work best for each individual.

We’re proud to be recognized by the American Diabetes Association (ADA) as an organization that ensures the highest quality patient care. Like most of our patients, you may discover that staying healthy is easier than you imagined—it just takes a little dedication to taking care of yourself.
