CATCH (Children's Access to Coordinated Healthcare)
Special care for children with complex medical needs.
Caring for children with multiple disabilities requires a high level of communication and coordination among care providers, medical specialists, social services, schools, local agencies, care centers, and families. Using a team approach to early intervention and overall health and development, CATCH manages each child’s care among all these specialists, keeping families informed and involved every step of the way. This coordinated effort helps us to keep these children as healthy as possible and reduces the strain and worry for their families.
We collaborate with families and care providers to create an individualized health plan for each CATCH participant. Our model establishes a “medical home” as the umbrella for coordinated care, overseen by physicians and specialized nurse practitioners—which means that most participants receive care right at their own house, and only visit the health center when necessary.
Supporting family members is an important part of our program. We provide a community resource coordinator who organizes special events for families, acts as a parent liaison, and performs home visits as needed for our Spanish-speaking families. In all ways, we strive to encourage each child and their family to be active members of our community.
Eligibility is open to children living in East Boston, Chelsea, Revere, Everett, Winthrop, and other Boston areas, with significant disabilities who require adaptive equipment and/or advanced therapies.
