Chronic Disease Education and Management (CDEM)

Working together to keep you healthy.

If you have a chronic disease, you aren’t alone. Staying as healthy as possible requires determination, information, and the support of professionals and others who know what you experience each day.

The Chronic Disease Education and Management program (CDEM) brings together a dedicated team of health providers to improve the health and vitality of our community members with chronic conditions, including diabetes, high blood pressure, high cholesterol, heart disease, depression, asthma, and cancer. CDEM provides social support, education, and individualized case management to assist these patients in making positive lifestyle changes and taking a proactive role in managing their health.

CDEM offers a Bilingual Diabetes Education Program, emphasizing education and self-management. Our classes help patients manage their disease through good nutrition and exercise, and the use of glucometers and medications. Classes are taught by our diabetes care nurse manager, nutritionist, and members of our pharmacy staff. As part of this program, we established a diabetes registry and standards of care within the health center, to ensure consistency in treatment and continued follow up. We’re proud to have recently partnered with the American Diabetes Association, and will further our efforts with the help of this valuable resource.

We have also established a Coumadin Clinic for patients who are taking anticoagulation medication. The clinic is staffed by nurses who have been trained in anticoagulation care. These nurses work as a team with primary care providers to oversee each patient’s treatment.
