The Elder Service Plan is centered at two facilities, Winthrop and Lewis Mall. The full interdisciplinary team at each site includes geriatricians, nurse practitioners, social workers, physical and occupational therapists, nutritionists, and other professionals.

Lewis Mall Our Lewis Mall facility is located at 225 Sumner Street in East Boston. This site houses an Adult Day Health Center and two social programs.

Winthrop The Winthrop facility, located at 26 Sturgis Street in Winthrop, provides an Adult Day Health Center, housing, and transitional housing for participants who need interim rehabilitation
